CONTACT
Contact Chelsea Latorre, Ph.D. to discuss testing needs, answer questions related to evaluation process, discuss fees and costs, and to learn about availability.
INSURANCE
Our office manager will verify your insurance benefits, inform you of your insurance coverage, and gather your availability for scheduling.
SCHEDULE
We will contact you to schedule an appointment. Testing is offered between 8 am-5 pm Monday through Friday.
PAPERWORK
You will receive a link to complete the new client paperwork on our secure client portal. Paperwork must be completed within 48 hours of your scheduled appointment to avoid rescheduling.
